New Patients

All new patients attending the Lyndhurst Dental Practice will be asked to fill out a full medical history as some medical conditions or medications can affect the mouth, and some conditions affect the way we need to treat you or organise your appointments. We will also ask you to fill in a new patient form. This provides us with valuable information about your previous dental experiences and if there is anything in particular you are worried about with your teeth.

Please click to view our medical history and patient assessment forms. If you would like to print these out at home, please fill them in and bring them with you to your appointment. However, if you would rather arrive 15 minutes before your appointment time, we will give you the appropriate forms to fill in at the practice. If there are any questions you are unsure of please discuss them with your dentist at the assessment appointment.

As a new patient we are likely to ask you quite a lot of questions. We want to get to know our patients and find out what is important to them. We also want to find out about occupation, lifestyle, diet and oral hygiene routines, as these can all have a big effect on the health of your mouth.

During the assessment of your teeth and mouth we have a lot to look at. We will check your jaw joints and your glands around your neck. Under lighted magnification we will carefully check your gums, the pink parts of your mouth for oral cancer as well as your teeth for holes, previous fillings and signs of abnormal wear. We also usually take x-rays of our new patients so that we can check the areas we cannot see directly in the mouth, for example under old fillings or between your teeth. We may also take photographs of your teeth so that we can show you how we see your teeth!

After a full assessment, we can then sit and discuss your treatment needs and options. As a team it is our job to tell you about any oral disease you may be suffering from and advise you on your options for combatting it. You can then make an informed decision about what is right for you.

Once a treatment plan is decided we will then provide you with a written estimate of the costs and appointments required.
